The membership card has been priced at Rs.
1500/- (price may vary from region to region).Every additional member added in
the same Family Health Card will be charged Rs.400/- extra.

applicable for two in a family is provided.

by two in a family is being offered for a mere Rs. 1999. Card+ Rs.1999
voucher combo = Rs 3500.
The health check up is carried out by Evalife
home collection team.
